Can it simplify ratios with decimals?

Simplification to a clean A to B form applies to whole numbers. For decimal inputs you still get the decimal and percentage forms.

What does the percentage mean?

It expresses the first value as a percentage of the second, so 16 to 9 reads as about 178 percent.
